DELEGATES PLEASE NOTE:  By submitting this registration you hereby confirm that you wish to register for the 2019 ICBMD Conference under the following conditions:

• Registration is for the full conference, from Monday, 2 September 2019 – Tuesday, 3 September 2019. Lunches are included.
• The personal details supplied in the registration form are the correct details of the delegate that will be attending the conference. If another person is to attend as a substitute, ICBMD will be informed prior to the conference.
• My institution/organization/department will transfer the applicable conference fee into the bank details detailed on the invoice  before 31 May 2019 (for Early Bird registrations) OR 31 July 2019 (for Full Fee registrations)
• If this registration is cancelled less than 14 days before the conference, OR if I do not attend the conference, I accept that the full conference fee will be payable.
• The Registration fee DOES NOT include accommodation and travel.
