DELEGATES PLEASE NOTE:  By submitting this registration you hereby confirm that you wish to register for the Hybrid 2021 ICBMD Conference under the following conditions:

• Registration is for the full conference, to be held on Thursday, 1 – 2 September 2021.
• The personal details supplied in the registration form are the correct details of the delegate that will be attending the conference. If another person is to attend as a substitute, ICBMD will be informed prior to the conference.
• My institution/organization/department will transfer the applicable conference fee into the bank details detailed on the invoice  before 31 May 2021 (for Early Bird registrations) OR 31 July 2021 (for Full Fee registrations) OR 31 August 2021 (for Late Registration)
• If this registration is cancelled less than 14 days before the conference, OR if I do not attend the conference, I accept that the full conference fee will be payable.
• Please note: This is a hybrid conference – the details will be communicated to you well in advance.
• The Registration fee DOES NOT include accommodation and travel.
• If you are bringing an accompanying person along, please make arrangements with your hotel directly. If the accompanying person is attending the technical sessions, they will need to register as a delegate.
